Mission Statement
The mission of the MWADRC is to create and sustain a “No Wrong Door” virtual partnership to assist elders and individuals with disabilities in their efforts to live independently.
The MWADRC promotes independence and choice and strives for a coordinated system for information and access for all persons seeking long term supports. In this manner, our mission is to minimize fragmentation and confusion for consumers.
Empowering individuals to make informed decisions about their long-term care support options and supporting individuals at high-risk of nursing home placement in their efforts to live in community settings are at the core of the MWADRC’s efforts.
What We Do
Our staff is trained to work with you and your family to connect you with supports and services that fit your current situation and preferences. We can talk with you on the phone, visit you in your home, meet with you in the hospital, or meet with you in a nursing home.
